Quick Answer

Acupuncture cannot reverse diminished ovarian reserve (DOR) or create new eggs. Some small studies, mostly from China, suggest acupuncture may be associated with improvements in markers such as FSH and antral follicle count, but the evidence is low certainty and has not shown more pregnancies or live births. Acupuncture for diminished ovarian reserve is best used as supportive care alongside a reproductive endocrinologist, including during IVF, to help with stress, sleep and overall wellbeing.

Key Takeaways

  • Ovarian reserve means the number of eggs remaining in the ovaries. It is a measure of egg quantity, not egg quality.
  • Women are born with all the egg-containing follicles they will ever have, and both egg quantity and quality decline with age.
  • DOR is usually assessed with AMH and an ultrasound antral follicle count (AFC). Day-3 FSH and estradiol are sometimes added, but they are less sensitive.
  • Ovarian reserve tests predict IVF egg yield, not natural conception or egg quality.
  • A 2023 meta-analysis of 13 small trials, all from China, linked acupuncture with lower FSH and higher AFC, but did not assess pregnancy rates.
  • A large 2018 trial of 848 women having IVF found acupuncture did not improve live birth rates compared with sham acupuncture.
  • DOR is not the same as primary ovarian insufficiency (POI), which is when the ovaries stop working normally before age 40.

What Is Diminished Ovarian Reserve?

Diminished ovarian reserve means the ovaries have fewer eggs remaining than expected for a woman’s age. It often arrives as a shock after a routine blood test.

Here is the most important thing to understand: ovarian reserve is about how many eggs are left. The ASRM committee opinion on testing and interpreting ovarian reserve defines it as oocyte quantity and treats egg quality as a separate concept. Egg quality depends mainly on age and whether an egg is chromosomally normal. We cover that topic in detail in our article on acupuncture and egg quality.

Every woman is born with her lifetime supply of eggs: about one million follicles at birth and about 300,000 by puberty, according to the American Society for Reproductive Medicine (ASRM). DOR means that natural decline is further along than expected. It does not mean pregnancy is impossible.

How is DOR different from POI?

Primary ovarian insufficiency (POI) is a different condition. The National Institute of Child Health and Human Development (NICHD) describes POI as ovaries that stop working normally before age 40. Periods often become irregular or stop. Women with DOR usually still have regular cycles, and the research on each condition is separate.

How Is Diminished Ovarian Reserve Diagnosed?

There is no single perfect test. Your doctor looks at several markers together, along with your age:

  • AMH (anti-Müllerian hormone): a blood test. ASRM notes AMH is a more sensitive marker than FSH and tends to fall before FSH rises. If AMH is your main concern, read our companion article on acupuncture for low AMH.
  • Antral follicle count (AFC): a transvaginal ultrasound early in the cycle that counts the small resting follicles in both ovaries.
  • Day-3 FSH and estradiol: a high FSH is specific for DOR but misses many cases. ASRM advises that estradiol should not be used on its own to screen for DOR, but it helps interpret FSH.

What these tests can and cannot tell you

AMH and AFC are good at predicting how many eggs may be retrieved during IVF stimulation. They are not good at predicting natural conception. In an NIH-funded study of 750 women aged 30 to 44 without known infertility, published by Steiner and colleagues in JAMA in 2017, women with low AMH were no less likely to conceive naturally than women with normal AMH. ASRM adds that these tests are only weakly linked to egg quality.

What Causes Diminished Ovarian Reserve?

Age is the most common factor. Other known contributors include:

  • Prior ovarian surgery, including removal of endometriomas (ovarian cysts related to endometriosis)
  • Chemotherapy or pelvic radiation
  • Genetic factors, such as a fragile X (FMR1) premutation or Turner syndrome
  • Autoimmune conditions, such as autoimmune thyroid disease
  • Smoking

In many women, no clear cause is found. That is not your fault.

What Does Research Say About Acupuncture for Diminished Ovarian Reserve?

The most relevant review is a 2023 systematic review and meta-analysis in Frontiers in Endocrinology by Lin and colleagues. It pooled 13 randomized trials with 787 women with DOR. All the trials were small, single-center studies conducted in China. Compared with control groups, acupuncture was associated with lower FSH, a lower FSH/LH ratio and a higher antral follicle count. The change in AMH was borderline. The authors reported high variability between studies, few blinded trials and too little data to assess pregnancy. They concluded the results should be “cautiously adopted.”

Put simply, some studies suggest acupuncture may influence ovarian reserve markers, but research is still early. No high-quality trial has shown it increases egg numbers or live births for women with DOR.

What about POI?

Acupuncture and moxibustion have also been studied for POI. A 2025 network meta-analysis of acupoint-stimulation trials reported improvements in hormone levels and menopause-like symptoms, but nearly all trials had unclear risk of bias and little or no blinding. These POI findings should not be applied directly to DOR.

How Might Acupuncture Support Women With DOR?

Researchers have proposed that acupuncture may influence pelvic blood flow and hormonal signaling between the brain and ovaries. These ideas are not proven.

Patients often tell us acupuncture helps them feel calmer and sleep better during a stressful time. The National Center for Complementary and Integrative Health (NCCIH) notes that relatively few complications have been reported when acupuncture is performed by a trained practitioner using sterile, single-use needles.

A traditional Chinese medicine lens

In traditional Chinese medicine, reproductive vitality is linked to the concept of Kidney essence, or Jing, which naturally wanes with age. Practitioners also consider the smooth flow of Qi and the nourishment of Blood. These traditional concepts guide how we choose points; they are not Western diagnoses and do not replace your medical workup.

Can Acupuncture Help Alongside IVF for DOR?

Many women with DOR move toward IVF. Acupuncture can fit alongside that care, with realistic expectations.

The largest trial, Smith and colleagues in JAMA (2018), randomized 848 women at 16 IVF centers in Australia and New Zealand. Acupuncture given during ovarian stimulation and at embryo transfer did not improve live birth rates compared with sham acupuncture. NCCIH also reports that a 2021 review of six studies found no difference in pregnancy or live birth rates between real and sham acupuncture during IVF. Women in the Smith trial did report feeling more relaxed.

How Does Acupuncture Compare With Other Options for DOR?

OptionWhat it doesWhat to know
Evaluation by a reproductive endocrinologistConfirms DOR, checks other factors and builds a medical planThe essential first step, especially if you are 35 or older
IVF with your own eggsStimulates the ovaries to retrieve as many eggs as possible in a cycleLower egg yield is common with DOR; some patients need more than one cycle
IVF with donor eggsUses eggs from a younger donorA medical option some patients consider, especially at older ages
SupplementsVaries by productEvidence is mixed; discuss any supplement with your doctor first
AcupunctureSupportive care for stress, sleep and wellbeing; may influence some markers in small studiesLow-certainty evidence; best used alongside medical care, not instead of it

What Does Acupuncture Treatment for DOR Look Like?

Your first visit includes a detailed history, a review of your labs and fertility plan, and a traditional pulse and tongue assessment. If you are new to acupuncture, our article on what to expect at your first appointment walks you through it.

We usually recommend weekly sessions over roughly three months. Research on follicle growth describes about three menstrual cycles, or roughly 85 days, for a follicle to grow from an early stage to ovulation, so a three-month window lines up with the eggs you may ovulate or retrieve. Our 3-Month Fertility Prep Program follows this approach.

Sessions include practical guidance on sleep, stress and nutrition. We do not promise specific results, and we always encourage you to keep your medical team informed.

When Should You See a Reproductive Endocrinologist?

See a reproductive endocrinologist (RE) first, or alongside acupuncture, if any of these apply:

  • You are under 35 and have been trying to conceive for 12 months, or 35 or older and trying for 6 months
  • You have a low AMH or AFC result, or a high FSH
  • Your cycles have become irregular or stopped before age 40
  • You have had ovarian surgery, chemotherapy or radiation, or have endometriosis

Time matters with DOR, so please do not delay medical evaluation.

Frequently Asked Questions

Can acupuncture reverse diminished ovarian reserve?

No. Acupuncture cannot create new eggs or reverse DOR. Some small studies suggest it may be associated with changes in markers like FSH and antral follicle count, but the evidence is low certainty.

Can I get pregnant naturally with diminished ovarian reserve?

Many women with DOR do conceive. Ovarian reserve tests are not good predictors of natural conception, so speak with a reproductive endocrinologist about your individual chances and timeline.

Is diminished ovarian reserve the same as early menopause?

No. DOR means fewer eggs than expected for your age, and cycles are usually still regular. Primary ovarian insufficiency is a different condition in which the ovaries stop working normally before age 40.

Does acupuncture improve IVF success for women with DOR?

Large IVF trials have not shown that acupuncture improves live birth rates. Many patients use it for stress relief and comfort during IVF.

How long should I do acupuncture for DOR?

We usually recommend weekly sessions for about three months, because follicles take roughly three cycles to mature before ovulation. Your plan is adjusted to your medical treatment timeline.

Is acupuncture safe while trying to conceive?

Acupuncture has relatively few reported complications when performed by a licensed practitioner using sterile, single-use needles. Keep both your acupuncturist and your doctor informed.
